Welcome to a presentation of the Project CRANE.
The comming demographic challenge will increase the pressure on the health care system and create a strong need for innovation in the field of self-care - in Arendal, in Agder, in Norway and in the rest of Europe. To address this situation, the EU project CRANE will focus on patients with chronic diseases in three district areas: Region Västerbotten in Sweden, Extremadura in Spain and Agder in Norway. CRANE will be based on the three regions in order to develop solutions that contribute to sustainable health and care services for the future.
CRANE will design a flexible and personal self-care model for patients with chronic conditions at home. Through analyzes of data from both the health services and from the citizens' own private technologies, CRANE will create new insights that can function as decision support for the health services and as a source of knowledge and competence for self-mastery for the citizens
